Anastomotic leakage is one of the most feared postoperative complications of pancreaticoduodenectomy (PD). The aim of this study was to assess the incidence of and factors that predict anastomotic leakage after PD. Patients who underwent PD were recruited at four centers. The primary endpoint was anastomotic leakage, defined as a clinical and/or radiological anastomotic disruption with a septic collection or abscess. Of the 106 patients who underwent PD, 37 (34.9%) developed anastomotic leakage. Multivariable analysis revealed four independent risk factors for leakage: soft-tissue contamination around the pancreas and bile duct [odds ratio (OR), 5.81; P = 0.008], body mass index (OR, 1.12; P = 0.001), and male sex (OR, 2.59; P = 0.019). The incidence of anastomotic leakage is 34.9% after PD.
